Hoja impresa en horizontal en tinta negra por una sola cara, con datos mecanografiados en tinta negra y cuño circular impreso. Todo enmarcado por orla dividida en triada con ornamentación simétrica. Certificado entregado por el brigadista general del comando de EE.UU., R.W. Buzzell del centro de entrenamientos de la infantería de Georgia, al soldado John M. Fradd (fallecido en el campo de batalla durante la WWII, y enterrado en Filipinas), por haber cursado exitosamente el curso de entrenamiento del 16 de octubre de 1944 al 27 de enero de 1945 en la especialidad del rifle.
