Hoja blanca mecanogradiada en tinta negra con membrete impreso en negro y anotaciones manuscritas en tinta negra. Tarjeta con emblema persa en tinta dorada y en el interior, en verde, rojo y oro, bandera de la República Islámica de Irán; fotografía a color de pequeño tamaño pegada en reverso, y anotaciones manuscritas en tinta azul. Carta que le escribe el jefe de la oficina del Ministerio del Exterior, Jafar Niknam, de la República islámica de Irán, a Pawel Pokrzywa, con motivo del envío de la firma del Dr. Ali Akbar Velayati, Ministro de Exteriores, junto con su fotografía y anotación que dice: "En el nombre del altísimo, espero que Dios nos permita ser buenos servidores".
